Η πρόθεση τύπου Zweymuller ή πρόθεση παραπλήσιου σχήματος και σύστασης, με επιφάνειες από βιοεμφυτεύσιμο τιτάνιο κυρίως σε συνδυασμό με κοτυλιαία πρόθεση με πτερύγια τύπου Βicon, έδειξε διεθνώς τα καλύτερα αποτελέσματα στα 20 χρόνια σε σύγκριση σχεδόν με όλες τις άλλες προθέσεις και έφθασε η επιβίωση της πρόθεσης στο καταπληκτικό ποσοστό του 98-100%, ιδιαίτερα στις περιπτώσεις που χρησιμοποιήθηκε στις αρθρούμενες επιφάνειες κεραμικό με πολυαιθυλένιο. Βέβαια με τα καινούργια ένθετα πολυαιθυλενίου [High cross linked, τύπου Rexpoll κ.λ.π.] σε συνδυασμό με βελτιωμένα ακόμη περισσότερο υλικά όπως κεφαλές από OXINIUM κ.λ.π. τα αποτελέσμα αναμένονται ακόμη καλύτερα.

Όταν στις αρθρούμενες επιφάνειες των αρθροπλαστικών ισχίου χρησιμοποιήθηκε αντί για κεραμικό σε πολυαιθυλένιο, κεραμικό με κεραμικό με σκοπό να μην υπάρχουν προϊόντα φθοράς πολυαιθυλενίου, εμφανίσθηκε σε λίγες ευτυχώς περιπτώσεις, το πρόβλημα του κριγμού κατά την βάδιση (ήχος τριξήματος) και σε ελάχιστες περιπτώσεις θραύση των κεραμικών υλικών.
Όταν σε κάποιες περιπτώσεις χρησιμοποιήθηκαν επιφάνειες από μέταλλο σε μέταλλο ( metal on metal ) εμφανίσθησαν προβλήματα φθοράς ( μετάλλωση, αλλεργίες κ.λ.π.).
Οι επιφάνειες metal on metal ( ΜΕΤΑΛΛΟ ΜΕ ΜΕΤΑΛΛΟ) χαρακτηρίζουν τις resurfacing αρθροπλαστικές ισχίου που μαζί με άλλα αίτια ( νέκρωση υποκείμενου οστού, κατάγματα αυχένος κ.λ.π.) εμφάνισαν πολύ μεγάλα ποσοστά αποτυχίας σε λιγότερο από 5 χρόνια με συνέπεια δύσκολες επανεγχειρήσεις για να αφαιρεθούν και να αντικατασταθούν τα εμφυτεύματα.

Most ASR hip replacement surgeries have been successful. However, data recently received by the company shows that more people than expected who received the ASR Hip System experienced pain and other symptoms that led to a second hip replacement surgery, called a revision surgery.
New data shows that five years after implantation, approximately 12% of patients (1 in 8) who had received the ASRresurfacing device and 13% of patients (1 in 8) who had received the ASR total hip replacement needed to have a revision surgery. For this reason, DePuy Orthopaedics is recalling its ASR™ XL Acetabular System and DePuy ASR™ HipResurfacing System.
This recall means additional testing and monitoring may be necessary to ensure your hip implant is functioning well. In some cases patients may need additional surgery. Abstract
Metal-on-metal hip replacements are the subject of much current debate. There is some evidence that there may be a hypersensitivity reaction, specific to metal-on-metal total hip replacements (THRs), which is associated with early failure of these prostheses. It has to date only been described in total replacements and not in metal-on-metal hip resurfacing. We present the case of a 68-year-old man who underwent bilateral metal-on-metal hip resurfacing for osteoarthrosis. The patient presented 6 months after surgery with pain and lateral thigh swelling. Pre-operative ultrasound and MRI demonstrated findings similar to those described in early failing metal-on-metal THRs, as well as evidence of lymphoreticular spread of metal debris. The operative findings included extensive aseptic soft-tissue necrosis. Histology revealed necrosis and a dense perivascular lymphocytic infiltrate along with metal debris within sinus histiocytes. The surgical, radiological and histological findings are similar to soft-tissue reactions described in metal-on-metal THRs.

Δεν ενημερώνονται σχετικά με το τι γίνεται διεθνώς; Η μία από τις δύο μεγαλύτερες εταιρείες με τις καλύτερες υποτίθεται RESURFACING αρθροπλαστικές στο ισχίο ήδη ανακάλεσε τις hip resurfacing αρθροπλαστικές!
Παράλληλα χρησιμοποιώντας τις λέξεις-κλειδιά στο διαδίκτυο (Google) Zweymuller, hip arthroplasty, Zweynuller threaded cups κ.λ.π. θα διαπιστώσετε τα άριστα και μακρόχρονα διεθνώς αποτελέσματα των Zweymuller ή παρόμοιου τύπου αρθροπλαστικών που εμείς χρησιμοποιούμε στους ασθενείς μας ακόμη και στους πολύ νέους σε ηλικία αντί της “resurfacing” ή “CUP, metal on metal” αρθροπλαστικής.
